After the Sugarbaker and Glehen groups developed CRS+HIPEC, it became established that long-term disease control is possible in selected cases. But this procedure requires experience. Results are unpredictable without dedicated fellowship training.
In my practice, a significant percentage of patients referred as having "inoperable peritoneal cancer" do become surgical candidates after careful PCI score calculation. I learned this specific assessment under Prof. Rau.
Complete cytoreduction followed by heated intraperitoneal chemotherapy. Best for PCI below 20, good performance status. 8-12 hours.
Laparoscopic, repeatable every 6 weeks. For patients not suitable for open CRS+HIPEC. Disease control and bridge therapy.
Systemic chemotherapy, targeted therapy via CION Tumor Board. Surgery is not always the answer — I refer when better options exist.
